Midfielders combine in midfield areas, to work ball wide to wingers to play around the defensive team, or ideally to the #9 to play through the defensive team.
The 3 pictures highlighted, are just combinations I have pictured prior to the session as examples.
However, my ideal scenario, is for the players to be creative and come up with their own combinations.
I may use one of more of these combinations as examples, with the hope then that they can again come up with their own solutions around those examples, or see the correct opportunities for my combinations.
I will hold back on showing these examples, as I want to encourage creativity in possession, and do not want players to be trying to force my combinations in inappropriate situations.

- Deepest lying midfielder recieves the ball, movement from second player to receive, 3rd man drops into hole to receive next pass (introduction of 3rd man run), lay the ball wide.
- 2nd player plays round the corner first time, to take advantage of early 3rd man run

Deep lying midfielders move wider, to create a passing lane through to the #10,
#10 drops in hole, one deep lyer moves to receive, the other makes run for the next pass (3rd man run),
Deep lyer plays first time ball through to utilise early run, then drops in central to offer defensive balance, with other deep lyer pushing forwards.

In this picture we see 2 different principles.
Rotation of players in order to lose tight marking and receive.
Ball goes across to other deep lyer, who combines with a one-two around the defender.
In order for one-two to work, second player must play first touch

Organisation
Neutral Players on each side of the Square. 3 v 3 central midfielders inside the Square (30 x 30)
Player receives ball from neutral player, and must combine with all 3 players touching the ball, and transfer the ball to a different neutral player. If successful, that team retains possession, receives again and plays through.
Defending team wins the ball and it goes out, they get possession from a neutral player.
Defending team wins it and gains possesion - Play to a neutral player for a point, now have posession.
Progression, (to encourage positive play forwards), 1 point playing to an adjacent neutal player, 3 points playing to the opposite neutral player
Regression. (if attacking team not showing defensive responsibility), Defending team wins it, and hits original neutral player - 3 points
Progression (to encourage certain play, e.g one-two), if players can complete task (one-two around a player) AND score, it is worth double points
Key Factors
Speed of movement/Thought - 3rd man runs, always think a pass ahead to stay ahead of defenders.
Speed of passing - Try to play with minimal touches to drag defenders out of positions.
Passing cominations e.g one-two
Types of pass - Surface, weight, curved passes, into feet/space, disguised
Rotation of players, can all fulfil each role, and this allows freer movement, and more difficult to mark.
Defensive responsibility in attack. It is important with this rotation,(at least) one player remains deep to offer balance should the game break down, making it game realistic.
